Through assessing, planning, implementing, coordinating, monitoring and evaluating available options and services, case managers help assure you receive excellent care from available resources.

 

Anyone may ask for Case Management at Care Resource.

 

Case managers will refer you to many community services including primary medical care, prescription drug assistance programs, housing assistance, food programs and many others.
